You have not yet won. No purchase or payment of any kind is necessary to enter or win this sweepstakes. A total of one (1) prize will be awarded consisting of $500 Visa Gift Card (ARV $500). Total ARV of all prizes: $500. Odds depend on the number of eligible entries received. Restrictions apply. The Sweepstakes commences on April 14, 2008 and ends on June 9, 2008. All entries must be received no later than June 9, 2008. Limit one (1) entry per person and email address.   1. How to Enter.
    Hosts of At the Death House Door (the "Film") screenings held between April 14, 2008 and May 27, 2008 can find a recap/entry form online at www.ifc.com/atthedeathhousedoor/screenings (the "Site"). Forms have also been made available to any group receiving a free At the Death House Door Screening in a Box Kit. On the recap/entry form, fill in your name, the name of your organization (if applicable), your telephone number and/or email address, the location where your screening event was held, the date of your event, the number of guests invited, the number of guests attended, the type of facility where your event was held, and any means used to promote your event (e.g. mailings, newsletters, listserv announcements or websites). You can host a screening by inviting one or two people over to watch the film or you can organize a larger community screening. To enter the Sweepstakes, you must return to Sponsor your fully completed recap/entry form ("Entry") by one of the following ways: (i) mail your Entry with proper postage to: ATDHD Recap Sweeps, 450 Park Avenue South, 5th Floor, New York, NY 10016; (ii) fax your Entry to "Attention: David Weiner" at fax number 212-426-7002; (iii) email your Entry to ATDHD@ifc.com; or (iv) follow the online entry instructions on the Site. The Sweepstakes begins on April 14, 2008 and ends on June 9, 2008. All online, email and fax entries must be received no later than 11:59 pm ET on June 9, 2008 and mail-in entries must be postmarked no later than June 9, 2008 and received by June 12, 2008. No automated entry devices and/or programs permitted. Sponsor is not responsible for lost, late, illegible, stolen, mutilated, incomplete, invalid, unintelligible, misdirected, postage-due, technically corrupted or garbled entries/requests or prize claims, which will be disqualified, or for problems of any kind whether mechanical, human or electronic. Proof of submission will not be deemed to be proof of receipt by Sponsor.
  2. Eligibility.
    Sweepstakes is open to all legal residents of the fifty (50) United States and District of Columbia, 18 or older at the time of entry. Limit one (1) entry per person. Employees of Sponsor and its affiliates, its dealers, its advertising and promotional agencies, its judging organization, manufacturers or distributors of Sweepstakes materials and their immediate families in the same household are not eligible. This promotion is intended for viewing in the United States only and shall only be construed and evaluated according to United States law. You are not authorized to participate in the Sweepstakes if you are not located within the United States. All federal, state and/or local rules and regulations apply.
  3. Random Drawing.
    Winners will be selected from among all eligible entries received in a random drawing. The random drawing will be conducted on or about June 15, 2008 by the Independent Film Channel or its agent, whose decisions on all matters relating to this sweepstakes are final. One (1) winner will be randomly chosen from a pool of all eligible entries received. Odds of winning depend on the number of eligible entries received. Winner will be notified by email or telephone on or about June 15, 2008. If winner cannot be contacted within five (5) calendar days of first notification attempt, if prize or prize notification is returned as undeliverable, if winner rejects his/her prize or in the event of noncompliance with these Sweepstakes rules and requirements, such prize will be forfeited and an alternate winner will be selected from all remaining eligible entries. Upon prize forfeiture, no compensation will be given.
  4. Prize.
    A total of one (1) prize will be awarded consisting of a $500 Visa Gift Card (ARV $500). Total ARV of prize: $500. All Gift Card terms and conditions apply. Prize is not redeemable by winner for cash value. Winner is solely responsible for any unspecified expenses and all federal, state, and local taxes and fees that may apply.
  5. General.
    VOID WHERE PROHIBITED BY LAW. SUBJECT TO ALL FEDERAL, STATE AND LOCAL LAWS AND REGULATIONS. Prize is non-transferable; no substitutions, exchanges, or cash redemptions except by Sponsor due to unavailability in which case a prize of equal or greater value will be awarded. All expenses on receipt and use of prize and any federal, state and local taxes on the prize are the responsibility of the winners. Winners may be required by Sponsor to execute an Affidavit of Eligibility and Liability Release and where lawful, a Publicity Release within ten (10) days of notification. Non-compliance may result in disqualification and award of prize to an alternate prize winner. By entering Sweepstakes, entrants agree to release and hold the Sponsor and its subsidiaries, affiliates, and agencies harmless from any and all losses, damages, rights, claims, and actions of any kind in connection with the sweepstakes. By participating in this Sweepstakes, entrants agree to abide by and be bound by these Official Rules, and understand that the results are final in all respects. By accepting the prize, winner grants to Sponsor the right to use their organization names, their personal names, likeness, hometown and biographical information, for any lawful purpose, without further permission or compensation, except where prohibited by law. By accepting prize, winners release Sponsor, and its advertising and promotion agencies, and their respective subsidiaries, affiliates, and agencies from any and all liability for any loss, harm, damages, costs or expenses, including without limitation property damages, personal injury and/or death, arising out of participating in this Sweepstakes, or the acceptance, possession, use or misuse of any prize and claims based on publicity rights, defamation or invasion of privacy and merchandise delivery. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. The Sponsor is not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any e-mail or entry to be received by Sponsor on account of technical problems or traffic congestion on the Internet or at any web site, any combination thereof, or otherwise, including any injury or damage to entrant's or any other person's computer related to or resulting from participation or downloading any materials in the Sweepstakes. Sponsor reserves the right, in its sole discretion, to cancel or suspend part or all of this Sweepstakes should virus, bugs, non-authorized human intervention or other causes beyond the control of sponsor corrupt or impair the administration, security, fairness or proper play of the Sweepstakes. In such event, Sponsor may award prizes in a random drawing from all eligible entries received up to the date of cancellation or suspension. Sponsor may prohibit an entrant from participating in the Sweepstakes or winning a prize if, in its sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Sweepstakes by cheating, hacking, deception, or other unfair playing practices (including the use of automated quick entry programs) or intending to annoy, abuse, threaten or harass any other entrants or Sponsor representatives.
    CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR(S) RESERVE(S) THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.
  6. Name of Winner.
    To obtain the name of winner (available after June 30, 2008), send a stamped, self-addressed envelope to: ATDHD Sweeps, 450 Park Avenue South, 5th Floor, New York, NY 10016. Requests must be received no later than July 31, 2008.
